Health and Nutrition Survey for Families

Dear Families,

Our school nurses and the district’s Farm-to-School Committee joined together to build a survey about health and nutrition in our classrooms and in our school cafeterias.  They would appreciate you taking time to answer a few questions about health and nutrition in our schools.
