Submit Health Challenge Registration Form for Health Course Waiver to the school Health Challenge Teacher

November 19, 2011
Take the Comprehensive Health Examination, 9:00-noon, Bellingham High School

March 2, 2012- Grades 9, 10, 11
Submit all performance task documentation

• Safe Living and Emergency Response
  • Adult CPR and/or First Aid Card

• Consumer Health
  • Community Service: 
    • SHOC Student Record
    • SHOC Student Reflection

                                Or

• Informational Interview:
  • Completed Information Interview Questions
  • Completed Student Reflection Sheet

• Social, Physical, Mental, and Nutritional Health
  • Individual Health Risk Analysis
