Overnight Field Trip Medication Request Form

Parent/guardians of students who will be taking medication outside of school hours and are participating in an overnight field trip are required to complete this form and submit it to their school five days prior to the field trip.

Free and Reduced Lunch Application

Ethnicity and Race Data Collection

This is a required form from the State Office of the Superintendent for Public Instruction and gathers ethnicity and race data.

This form is required with registration to verify what neighborhood school the student should attend based on where they live.

Student Rights and Responsibilities

3231 Form – Agreement of Student Information Release

It is the district’s policy that the release of student records is for school-related purposes only, and will not be used for commercial purposes. School-related purposes are those purposes which the district designates as useful to the operation of the school program.

2320 Form-2 – Approval in Concept

This is a form for staff to complete for out-of-state, overnight trips of more than one night, and/or trips that involve fundraising.

Pursuant to Bellingham Public Schools policy 2320, this staff form for approval must be completed for all student and ASB non-WIAA, school day, out-of local area, overnight trips, out-of-state and out-of-country travel including summer travel, etc.
